Not quite the same. Reincarnation is done usually on birth, but there are stories and anecdotes of the next life being born before the previous dies, or in contrast, sometimes after. And as for passing on one's soul to another, I've never seen it work quite like that... even if I wouldn't be surprised if such a thing existed in a special case or two.
